• Valid identity document of policyholder

•  Notification of your intent to claim benefits within 6 months of the incident
• Completed Claim form by the claimant
• Original policy documents, policy schedule or any endorsements
• Valid identity document of both the insured and beneficiary or trustee
• Certified copy of death certificate
• Proof of relationship
• In the event of death at home- Police Extract
• In the event of accidental death- Police report
• Any other information that Metropolitan may require or deem necessary at the time.
